Coastside Clinic will be open in midday this week and next for records retrieval [UPDATED]

UPDATE: Patients are reporting that the CFMC is not making copies of medical records today, or perhaps this week.

Former patients may retrieve your records from the Coastside Family Medical clinic through the following process

1. Contact the clinic.

  • Call 650-712-2311 or 2324 leave a message with your full name, date of birth and return phone number.
  • Email your request to [email protected] Please include your full name, date of birth and phone number.
  • Stop by by the clinic and fill out a request for release of information (copies of the release are posted at the front door of the clinic).

2. Allow 2 to 3 days for your chart to be retrieved.

3. Pick up your records.  Patients must bring a valid form of identification to pick up records. CFMC will be open for record requests and pickup:

  • March 18 to 20, 10:00am to 2:00pm.
  • March 23 to 27, 10:00am to 1:00pm.

Well, certainly not today, and possibly not the rest of the week, or even next week. It seems the defunct CFMC has run into legal issues about whether they can hand out originals, and they don’t have the money or assets for making copies. It is in the hands of lawyers at this point, with no estimate of when it will be resolved. If you are part of a medical group, like Mills Peninsula, you can ask them to retrieve your original records and keep them on file for you.

Isn’t it amazing that the members of the board were able to get their records right away?

Today in Montara I spoke to a friend.  He was unaware that CFMC had stopped operating on Friday.  He was particularly shocked at the closure because he was a patient at CFMC.  I would encourage an outreach program by CFMC to each family it has served.

The following has been written in an effort to clarify the state of the medical charts at CFMC.  Please let us know if you have questions as we would like to avoid any misunderstandings.
If you had medical insurance with MPMG HMO, your chart was transported by MPMG to their medical records department and will be delivered to your  new primary care provider upon request by said provider.  MPMG serves as a temporary custodian of your medical chart.
If you had medical insurance with HPSM, your chart was transported by the county to their medical records department in san mateo.  It will be delivered to your new primary care provider upon request by said provider. HPSM serves as a temporary custodian of your medical chart.
If you have some other form of insurance, or lack of, you need to choose a new primary care provider.  Your new provider should ask for your records.  The actual medical charts will be held at CFMC until the bankruptcy trustee(who will be custodian of the records) has them moved to storage.  It is suggested patients do this within two weeks.  Any cost for shipping of the chart must be paid for before shipped.
If you need some of your medical chart copied due to unusual circumstances, you can contact the clinic and complete a medical release form .  You will be charged a fee per page and can expect to receive them within 15 days of your request.  In order to streamline this process, we are requesting patients pay $10 when they drop off the form; change will be made when the records are picked up or if you owe monies, the remainder will be due before the records are released. 
If you wish to receive medical records of someone who is deceased, you must provide an original death certificate for a CFMC employee to view at the time of the request.
